The platform facilitates secure and private transmission of medication information between health professionals, patients and researchers.
A best possible medication history (BPMH) is made available to inform prescribing and medication reconciliation activities during hospital admissions, discharges and other transitions of care. Patients will also have access to a comprehensive medication history to share with their caregivers and care providers in their circle of care.
HealthChain enables standards-based connectivity between disparate health databases and stakeholders utilizing one of the most advanced FHIR-based data management and clinical data exchange infrastructure available.

Nick is Chief Executive Officer at HealthChain is responsible for setting the vision for the company and defining the overall strategy including go to market approach and growth targets. He brings to the company over 30 years of healthcare industry experience; a passion for safer and accurate medication use; and credentials as a pharmacist, healthcare administrator, consultant and entrepreneur.
Daniel Renton
Daniel is Chief Revenue Officer at HealthChain is responsible for all revenue-related activities across the company’s client base, tasked with the goal of continuing the adoption and growth of HealthChain’s technology and strategic partnerships across the Healthcare ecosystem. He is also the Healthcare lead for Inforica Inc.

Toby is the Chief Technology Officer at HealthChain is responsible for overseeing the development and dissemination of technology for external customers, vendors, and other clients to help drive our business.
